About: You pay the very same quantity monthly, whether or not you get care. In our example, whether you pick to have surgery or not, your monthly premium stays the same. Depending upon the type of health insurance you have, you may pay your regular monthly premium yourself, your employer may pay your premium for you or you may share the expense (with your part coming out of your income. Out-of-pocket optimum is the most you'll spend for your healthcare for the year. Once you've reached your health insurance plan's out-of-pocket optimum for the year, your plan will pay 100 percent of any other covered medical costs you have for the rest of the year.
